Family Support Specialist
The role of Family
Support Specialist is to help families develop healthy attachment, create
positive adult/child relationships, and achieve economic, physical, social and
emotional security. This position is
responsible for meeting with families in their home and community settings,
providing links to resources, parent education, and support of family goals.
ESSENTIAL
DUTIES AND TIME ON TASK EXPECTATIONS
Direct Client Support (60%)
- Establish
professional relationship with referred families
- Conduct visits in-person
and remotely during times that are convenient to parents to provide
program specific resources designed to strengthen families
- Support families to
identify their strengths and areas of concern
- Partner with
families to set goals and achievable action steps
- Provide case
management and connect families to community resources
- Provide
opportunities for families to socialize and learn with peers
- Facilitate
groups, workshops and classes in-person and virtually
- Provide evidence-based curriculum to enrolled families
- Support recruitment for current programs
Resources/Collaboration
(15%)
· Represent TLC at community meetings as assigned
· Build a rapport with community partners to
facilitate referrals and follow-up
· Collaborate with other TLC programs including Recovery, Youth and Rural Outright
· Participate in multi-disciplinary teams with
partner organizations
Documentation (15%)
- Complete required
electronic documentation and program specific forms thoroughly, accurately
and on time using Apricot, QuickBase and other data platforms as required
by contract
- Follow and adhere
to agency policies related to securing confidential information
- Accurately
report time worked using TimeWorksPlus
- Submit
monthly report of activities to Program Director
Meetings/Training/Supervision (10%)
· Participate in supervision and self-evaluation
· Participate in required program specific
trainings and approved professional development
· Meet time on task expectations
REQUIREMENTS
· Bachelor's Degree in related field
· At least two years related experience preferred
· Reliable transportation and ability to travel
throughout Sullivan County, NH daily
· Proficient with Microsoft Office Suite and other
software systems
· Excellent verbal and written communication
skills
· Excellent interpersonal and customer service
skills
· Familiar with community resources available in
Sullivan County, NH
· Strong analytical and problem-solving skills
